Guidance for completing your DSE Assessment

Full guidance on the safe use of display screen equipment is available on the Health & Safety Blackboard site (select 'Safe Display Screen Equipment use' from the left hand menu of the site homepage), the diagram link below provides a one stop visual aid to setting up your workstation.

Please ensure you have attempted to set up your workstation properly before completing the assessment

You will be sent an email of your completed assessment, also:

If you answer no to questions, which might require replacement equipment furniture, your line manager will automatically be emailed with your request.

If you answer no to questions, which might require further investigation of your working environment in respect of temperature, ventilation or lighting, Estates and Property Services will automatically be emailed with your request.

If you answer no to questions, which might require replacement IT equipment, IT Services will automatically be emailed with your request.

If you require an ergonomic assessment to be carried out by our specialist, your line manager will be provided with a copy of the report to ensure the advice provided can be implemented.

If you answer no to questions, which indicate you need help setting up your workstation, or that you have a medical condition, your Health and Safety Coordinator will automatically be emailed with your request.

A copy of all completed forms is also sent to Health, Safety and Wellbeing.

You are entitled to a free eyesight test if you meet the requirements of the DSE Code of Practice. Further information about how to claim your entitlement is available on the Safety Documents Page.

Please confirm by ticking this checkbox that:

  • you have set up your workstation as well as you are able to,
  • you have understood who will be notified of your needs before continuing with the assessment
  • and are aware of your entitlement to a free eyesight test if you meet the requirements of the DSE Code of Practice
